Taking Charge of Your Health

As we start a new year it may be time to take stock of your health, explore VA programs for healthy aging, start a healthy habit, or simply learn more about the wide range of topics and resources found at www.va.gov/Geriatrics.

Gerofit

Gerofit is a proven exercise program that promotes health and wellness for Veterans. Gerofit helps Veterans to stay active by using a variety of strength and aerobic exercises in group classes like tai chi, dancing, walking, and balance – in person or in your own home.

Talk with your primary care provider before starting an exercise program and about joining a Gerofit program if there is one near where you live.

Whole Health

Whole Health is VA’s approach to care that supports your health and well-being. Whole Health centers around what matters to you, not what is the matter with you. This means your health team will get to know you as a person, before working with you to develop a health plan based on your values, needs, and goals.

Find out how you can practice self-care from home through the #LiveWholeHealth videos. This series highlights Whole Health resources—such as video sessions for yoga or meditation—that you can follow along at home to live healthier and happier.

Geriatric Evaluation

Geriatric Evaluation is a health care visit with a team of experts that results in a treatment plan aimed at helping Veterans with complex care needs:

  • Maintain good health
  • Improve quality of life
  • Reduce the need for hospital stays
  • Reduce the need for long term care services
  • Remain independent using VA and community resources

  • Home and Community Services
  • Residential Settings and Nursing Homes
  • Paying for Long Term Care
  • Health, Fitness and Rehabilitation
  • Making Decisions and Advance Care Planning
